Request a consultation

To request a consultation and use our services, you must:

1. Be 21 years of age or older.
2. Be located in the province of Québec.
3. Have a valid health insurance card (RAMQ).
4. Have a copy of the referral provided to you by your family doctor.

Before getting confirmation of your appointment, you must complete the following steps:

1. Read and accept the Consent Form.
2. Upload a copy of the referral document provided to you by your family doctor. Please note that we do not process any consultation requests by fax.
3. Complete all of the pre-visit questionnaires.
These take approximately 30 to 45 minutes to complete, but can be paused and completed in several sessions, if needed.

Once you have completed all the steps above, you will receive a confirmation message by SMS / email.

Before your appointment, relevant information concerning your consultation, including the videoconference link to use at the time of your appointment, will be sent to you.
